29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about February 16, 2024, and dates prior at the Mazak machining center and other equipment to include CNC machining centers and lathes. The employer had not developed or documented procedures to be utilized for the control of hazardous energy to prevent the unexpected start up and energization of equipment. This exposed employees caught in, struck-by hazards and crushing hazards as well as electrical shock injuries.
