29 CFR 1926.416(a)(1): The employer permitted an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work and was not protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means: a) On February 9, 2024, employees performing carpentry work to a new structure located at 2503 - 2511 W Farragut Ave., Chicago, IL 60625, were permitted to work in proximity to energized overhead powerlines and were not protected against electric shock. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches above the walking/working surface, were not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: a) On February 8, 2024 , employees located at 2503 - 2511 W Farragut Ave., Chicago, IL 60625, were engaged in framing carpentry work and were not protected from fall hazards while working around unguarded wall openings on the third floor. No abatement certification or documentation is required for this item.
