29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Jobsite: 628 Telegraph Road, Coatesville, PA 19320 - On or about February 16, 2024,the employer did not ensure that powered industrial truck operators operating powered trucks such as, but not limited to, Crown WP 3000 Series forklift truck (Model WP3035-45, S/N 7A298625) successfully completed the training and evaluation to ensure operators have the knowledge and skills needed to operate the powered industrial trucks safely. Training shall consist of a combination of formal instruction, practical training, and evaluation of the operator's performance in the workplace.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) Jobsite: 628 Telegraph Road, Coatesville, PA 19320 - On or about February 16, 2024, the employer did not ensure that flexible cord sets were inspected for external defects prior to each shift or use. Employees used flexible cord sets with external defects including but not limited to damaged wire insulation and exposed wires, thereby exposing employees to electrical hazards.
