29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about February 6, 2024, an employee was installing shingles on a residential roof under construction using a pneumatic nail gun without the use of eye protection, exposing employees to struck by hazards.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about February 6, 2024, employees were installing shingles on a residential roof under construction at heights above approximately 6 feet without the use of fall protection, exposing employees to a fall hazard.

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level:  On or about February 6, 2024, employees accessed the second-story roof by means of an extension ladder that was set up on the inclined first-story roof, exposing employees to a fall hazard.
