29 CFR  1910.178(l)(6): The employer did not certify by name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation that each operator had been trained and evaluated as required by this paragraph (l):  6 Picker Rd, Sturbridge, MA: On and before September 12, 2023, the employer had not prepared a written certification record that documented that PIT operator employees had been evaluated which includes the name of the operator, the identity of the person(s) who performed the evaluation, and the date of the evaluation.
