29 CFR 1910.243(c)(2): Cup wheels (types 6 and 11) were not protected by any of the guarding methods listed in subparagraphs (i) through (iii): a) On or about February 11, 2024, at the Steam to Methane Reformer PAU1 Unit, the guard on an angle grinder with a Type 11 cup wheel, being used by employees to grind metal, did not cover the spindle, nut, and flange, nor did the guard fully enclose the sides of the grinding wheel.

1910.243(c)(2) Cup wheels. Cup wheels (Types 6 and 11) shall be protected by: 29 CFR 1910.243(c)(5)(i): Immediately before mounting, all wheels were not closely inspected and sounded by the user (ring test, see Subpart O, 1910.215(d)(1)) to make sure they have not been damaged in transit, storage, or otherwise. The spindle speed of the machine was not checked before mounting of the wheel to be certain that it does not exceed the maximum operating speed marked on the wheel: (a) On or about February 11, 2024, at the Steam to Methane Reformer PAU1 Unit, cup wheels used on angle grinders by employees grinding metal were not ring tested prior to use, nor was the spindle speed of the machine checked to ascertain that it did not exceed the maximum operating speed marked on the wheel.
