On or about February 20, 2024, and at times prior thereto, employees performing concrete work in a trench with accumulated water several inches deep while operating electric powered hand tools energized by a damaged/worn electrical cord laying in the water were exposed to electrical shock hazards.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. On or about February 20, 2024, located in the 3900 block of Conveyor dr. (FM 917) Johnson County, employees were working in a trench that was approximately over six (6) feet deep and 35 feet long by 20 feet wide and between a six-foot-tall concrete wall frame and the street side vertical wall which any safe egress was blocked with two-by-fours and sheet lumber exposing employees to entrapment hazards.

29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation. On or about February 20, 2024, and times prior thereto, employees were working in a trench where water accumulated without sufficient precautions taken, no monitor and no water pump were found near the trench while employees worked in conditions where water existed and signs of soil collapsing existed.
