(a) On or about February 21, 2024, at 1529 Creekview Court, Oconomowoc, WI 53066, the employer did not ensure that employees working from a Skytrak 6043 Telehandler at an approximate height of 19ft were protected from falls to the lower level with fall protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) On or about February 21, 2024, at 1529 Creekview Court, Oconomowoc, WI 53066, employees conducting framing work on a second story of a new build residential home were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 19 feet on the front and back of the house.
