29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) Worksite, stairwell concrete form: On or about February 20, 2024, an employee was utilizing a portable extension ladder to access the top level of the concrete form, which was secured to two concrete blocks and the break in elevation to access the ladder was approximately 48 inches above the ground.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device was not provided to assist employees in mounting and dismounting the ladder: a) Worksite, stairwell concrete form: On or about February 21, 2024, an employee was accessing the top level of a concrete form with the use of a portable extension ladder and a grasping device was not provided to assist the employee. The employee was exposed to a fall hazard.
