29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: a) For room number 4 at the T6 refrigerated warehouse facility, on or about 02/21/2024, the employer exposed employees to egress hazards, in that, the room's exit door was frozen shut from the inside and unable to be opened without several attempts to jar it free from ice accumulation.

29 CFR 1910.178(a)(4):Modifications or additions which affect capacity and safe operation of powered industrial trucks were performed by the employer without the manufacturer's prior written approval, and capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly: a) For the refrigerated warehouse facility T6, On or about 02/21/2024, the employer exposed employees to potential fall hazards, in that, a man-basket attachment was used with forklifts to elevate personnel, and the forklift manufacturer's approval had not been obtained to use the man-basket. Capacity/operation data plates were not updated to account for the man-basket attachment.
