Roof equipment  On or about February 21, 2024, and at times prior, the employer did not implement effective emergency shut down procedures of the ammonia refrigeration equipment, in that the operating procedures did not provide clear instructions for shutdown of ammonia refrigeration equipment on the roof in the event of an uncontrolled release.
