29 CFR 1926.251(a)(1): Rigging equipment for material handling shall be inspected prior to use on each shift and as necessary during its use to ensure that it is safe. Defective rigging equipment shall be removed from service. a. Mid-Ohio Pipeline Company located at 13757 Miller Road, Johnstown, OH 43031: On or about February 21, 2024, during the construction/installation of a natural gas pipeline, the employer did not ensure that defective rigging equipment was removed from service when discovered. Rigging equipment (lifting chains with hooks) used for lifting trench boxes into trenches had defects such as but not limited to missing hook safety latches and was left in service which exposed employees to struck-by hazards.

Mid-Ohio Pipeline Company located at 13757 Miller Road, Johnstown, OH 43031: On or about February 21, 2024, the employer did not ensure employees using portable power cords to provide power to an portable electrical submersible pump were protected from electrical hazards. Employees used a portable power cord without a ground prong. The following condition exposed employees to shock hazards.
