29 CFR 1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. a. On or about February 21, 2024, the employer did not ensure employees required to wear respirators were fit tested for activities such as, but not limited to, cleaning chemical storage and measurement tanks which required the use of a respirator.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. On or about February 21, 2024, employees were exposed to unguarded pinch points and moving parts at the sampling machines while working at the glove puller position. The conditions exposed employees to caught in hazards.
