29 CFR 1910.30(a)(1):The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. On or about February 5th, 2024 Location: 89 Baldwin Rd, Bedford Corners, NY 10549 a) Employee was engaged in cleaning the roof and gutters and accessing the roof with an 28 ft Louisville extension ladder that was improperly set-up, The ladder was not vertical. The employee jumped off the ladder from a height of about 10 ft as the ladder began to fall to the side. Employee has not received ladder training.
