29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. On or about February 22, 2024, at shop floor, the employer did not obtain written approval from the manufacturer prior to using a slab attachment on powered industrial truck which affect its capacity and safe operation. The slab attachment is used by employee(s) to move stone slabs.
