29 CFR 1910.141(a)(3)(i):All places of employment shall be kept clean to the extent that the nature of the work allows. a) On or about February 8, 2024, the employer did not ensure the work floor around line 10 was kept clear in that the scrap plastic from the line (trim strip) was pulled along the floor for approximately 10ft where employees worked between the line and the grinder feed at the front of the line.

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On or about February 2, 2024, the employer did not ensure the slitter knives on line 1 were guarded to prevent employees from touching the cutting blade during operation in that the slitter knife guards were not secured in place and could flip up. b) On or about February 8, 2024, the employer did not ensure the slitter knives on line 11 were guarded to prevent employees from coming into contact with the blade.

29 CFR 1910.212(a)(2):General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. a) On or about January 27, 2024, the employer did not ensure the kickback guards on the DeWalt table saws used by employees to make plastic retains did not cause a hazard in that an employee's right index fingertip was amputated when it was caught by the serrated kickback guard.
