On or about February 22, 2024, at the main warehouse employees were exposed to slip, trip and combustible dust hazards as there was caked on sawdust on the walls and multiple small piles of sawdust on electrical equipment and on and around the floor of machines which were not cleaned adequately. 2. On or about February 22, 2024, at warehouse #2 employees were exposed to slip, trip and combustible dust hazards as there was a pile of saw dust on an electrical outlet which was not adequately cleaned up.
