29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On February 22, 2024, on the north side of the construction site, employees engaged in roofing work without fall protection were exposed to unprotected fall hazards of approximately 14 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On February 22, 2024, on the north side of the construction site, employees using extension ladders that did not have the side rails extended three feet over the upper landing surface were exposed to potential fall hazards greater than six feet.  a) On the north side of the construction site, employees using an extension ladder that did not extend 3 feet above the landing, b) On the west side of the construction site, employees using an extension ladder that did not extend 3 feet above the landing.

29 CFR 1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On February 22, 2024, on the north side of the construction site, employees were installing roofing tiles using pneumatic nail guns without wearing safety glasses were exposed to eye injuries.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.   On the construction site, the employer did not develop or provide fall protection training to the employees engaged in roofing work while working over 6 feet to recognize and eliminate hazards such as fall protection.
