On or about 2/23/2024, at 8148 NW 74th Ave Medley, FL 33166, a JLG G6-42A all terrain forklift with mechanical issues such as leaking oil and bended load backrest located at the lab area was not taken out of service until safe to operate. b. On or about 2/23/2024, at 8148 NW 74th Ave Medley, FL 33166, a JLG 2646ES Scissor lift with mechanical issues such as worn tires, and missing/bended rails located at the lab area was not taken out of service until safe to operate. c. On or about 2/23/2024, at 8148 NW 74th Ave Medley, FL 33166, a Toyota 10026 forklift with worn tires and bended load backrest located at the lab area was not taken out of service until safe to operate.

Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly: On or about 2/23/2024, at 8148 NW 74th Ave Medley, FL 33166, forklifts with an attachment not approved by the manufacturer was being used to perform welding work at the lab area.
