29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Worksite, underneath 2020 Kenworth T880 truck: On or about February 21, 2024, the employer did not develop procedures to control potentially hazardous energy sources such as, but not limited to potential energy of a running vehicle, and kinetic energy of a rotating PTO shaft located on a Kenworth T880 work truck. Employees exposed to crushing and caught in hazards.

The maintenance that was being conducted was not completed by an authorized employee.
