On or about 2/23/2024, lockout/tagout procedures were not developed or utilized to protect employees from electric and pneumatic energy when performing servicing and maintenance tasks, such as adjusting and tightening the tension on low hanging foam material on the rollers of the Island Nip Axis 3 on the Delta 2 machine.
