29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards that enabled each employee to recognize the hazards of falling and that trained each employee in the procedures to be followed in order to minimize these hazards: a) Job site, 4279 Ohio River Boulevard, Pittsburgh, PA: On or about February 23, 2024, prior to performing roofing activities, the employer did not ensure that employees were provided with proper fall protection training, exposing them to fall hazards of at least approximately 12 feet. Deficiencies noted during the inspection included, but were not limited to, failing to have leg straps in use while wearing a harness and allowing too much slack in fall protection ropes, allowing the workers to fall off of the roof.

29 CFR 1926.20(b)(2): The employer did not have a program that provided for frequent and regular inspections of the job sites, materials, and equipment to be made by a competent person: a) Job site, 4279 Ohio River Boulevard, Pittsburgh, PA: On or about February 23, 2024, during roofing activities, the employer did not ensure that a system was in place to identify and correct safety issues. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection systems/devices correctly, not having an electrical service drop covered with insulative material and not having proper labeling on a ladder that was used to access the roof.
