29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:   location: 6th floor on or about: 2/14/24  a) Employees were using a ladder placed in a duct opening to access the 6th floor. The employees, using the access and working around the 32"X40" opening, were not provided with fall protection. The employees were exposed to a fall of 18'8".    NOTE: Because abatement of this violation is already documented in the inspection casefile, the employer need not submit certification or documentation of abatement for this violation as normally required by 29 CFR 1903.19.
