Jobsite, 903 Rossmore Avenue, Pittsburgh, PA 15226: On or about February 23, 2024, the employer did not have in writing any training certificates to verify that employees were trained on when and how to use fall protection. Employees were exposed to an approximately 25 foot fall hazard while installing a metal roof.

29 CFR 1926.1053(b)(1): Where portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a.) Jobsite, 903 Rossmore Avenue, Pittsburgh, PA 15226: On or about February 23, 2024, employees were using a portable ladder to access the top and bottom roof. The ladder side rails did not extend at least 3 feet above the upper landing surface. Employees using the ladder to access the roof were exposed to a fall hazard of approximately 25 feet to the ground level.
