29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On or about February 20, 2024, at the Amada Press brakes facility production area, the employer did not develop and implement specific written procedures for the control of potentially hazardous energy for equipment on which employees were performing service and maintenance activities such as die setting operations. Employees were exposed to amputation and caught-in hazards on machines and equipment to include the Amada Press Brake machine.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment (PPE). If such hazards were present, or likely to be present, the employer did not: (i) Select, and have each affected employee use, the types of PPE that would protect the affected employee from the hazards identified in the hazard assessment; (ii) Communicate selection decisions to each affected employee; and (iii) Select PPE that properly fit each affected employee: a) Behind the building, on or about February 20, 2023, and at times prior, the employer exposed employees to chemical burns, in that the employer did not provide appropriate personal protection equipment (PPE) for the body when changing propane tanks.
