29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe: a) Trench at 320 Airbus Way, Mobile, AL: On or about February 23, 2024, and at times prior, the employer exposed employees to struck-by and crush-by hazards, in that; employees were allowed to use a hook attached to an excavator that was missing its safety latch and not inspected and taken out of service, replaced, or repaired.

29 CFR 1926.251(a)(4): Special custom design lifting accessories for material handling were not marked to indicate the safe working loads and were not proof tested prior to use to 125 percent of their rated load(s): a) Trench at 320 Airbus Way, Mobile, AL: On or about February 23, 2024, and at times prior, the employer exposed employees to struck-by and crush-by hazards, in that; employees were allowed to use a steel prybar as a lifting accessory while moving prefabricated concrete pipe sections weighing 2,000 pounds that did not have markings to indicate safe working loads and was not proof tested.
