29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  At the main facility, employee(s) engaged in electrical and carpentry activities working on a catwalk suspended behind a wall to the south of the performance stage were exposed to fall hazards in that the catwalk did not have guard rails or other form of fall protection. The employer provides a personal fall arrest system but the employer does not enforce its mandatory use. Use of the personal fall arrest system is voluntary.

29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.   At the main facility, employee(s) engaged in electrical and carpentry activities working on a catwalk suspended behind a wall to the south of the performance stage were exposed to fall hazards in that the employer did not provide training to employee(s) on the hazards of working 4 feet or higher on the catwalk.
