29 CFR 1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) Residential structure located at 113 Riverside Lane, Woodstock, GA 30188, Roof: On or about 02/26/2024, the employer exposed employees to struck-by hazards while operating pneumatic roofing nail guns, in that protective eyewear was not provided to employees operating these tools at 130 PSI.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) Residential structure located at 113 Riverside Lane, Woodstock, GA 30188,  Roof: On or about 02/26/2024, the employer exposed employees to fall hazards of approximately 12-feet, 7-inches to the lower level, in that a fall protection system was not provided to employees engaged in roofing installation activities.

29 CFR 1926.1053(b)(1): Where(a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  a) Residential structure located at 113 Riverside Lane, Woodstock, GA 30188,  Roof: On or about 02/26/2024, the employer exposed employees to fall hazards of approximately 12-feet, 7-inches to the lower level, in that the ladder used to access the roof of the structure was not extended at least 3-feet above the upper landing surface.
