OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the underhung crane and hoist did not receive an annual inspection. as required by ASME B30.16-2003, Section 16-2.1.3(a):  Facility:  For 2022 and 2023, the employer did not have an annual inspection conducted on the underhung Abell-Howe (Serial # 78-11972-01) crane and the R&M Hoist (Serial # 1781819601) 3 ton hoist.   ASME B30-16-2003 Overhead Hoists (Underhung), Section 16-2.1.3(a) requires an annual inspection with a written report of the findings of underhung cranes/hoists.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)  Facility:  On or about 2/26/2024, the Pratt & Whitney Lathe and Summit Lathe were not fitted with chip/chuck guards.  In addition, the lathe operator would use strips of emery cloth to sand/polish a piece as it was turning in a lathe.  This activity exposes the employee to the hazards of rotating lathe chucks.  (b) Facility:  On or about 2/26/2024, some of the non-cutting portion of the horizontal band saw blade was not guarded from employee contact.

The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired:  (a) Facility:  On or about 2/26/2024, there was an approximately 10 inch by 12 inch floor opening cut in the top of the oil reservoir for the oil system for the plant machinery.  Employees would sometimes walk on the top of this oil reservoir and could easily slip into the 10 inch by 12 inch opening.

The one spreader on the ladder was homemade and it was only connected tothe ladder at one point.
