On or about 2/26/2024, a) An employee working from an aerial lift at approximately 20' from the ground was not tied-off with a body belt or lanyard. Employee was installing the support prior to hanging of a commercial sign. FIVE STARS AWNINGS AND SIGNS INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR ESSENTIALLY SIMILAR STANDARD, 1926.453(b)(2)(v), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1547727, CITATION NUMBER 1, ISSUED ON 2/9/2022, NOTE: FINAL ORDER DATE 3/9/2022.
