29 CFR 1926.416(e)(1):Worn or frayed electric cords or cables were used: At the temporary jobsite located at 25106 W 177th Terrace, Gardner, Kansas, employees engaged in residential framing activities were exposed to electrical hazards in that the extension cords providing 220 VAC power were significantly damaged. The insulation had pulled away and frayed, exposing the conductors.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): At the temporary jobsite located at 25106 West 177th Terrace, Gardner, Kansas, employees engaged in residential framing activities were exposed to fall hazards of approximately 11 feet 6 inches from the eave of the roof to the ground below in that fall protection was not utilized.
