29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew: a) 325 Fulling Mill Road Middletown, PA 17057, hazardous energy control did not facilitate group lockout by utilizing hardware that permitted the application of multiple locks, on or about February 27, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: (a) 325 Fulling Mill Road Middletown, PA 17057, multiple authorized employees were preforming a block line mold change on a conveyer line, and each authorized employee was not able to affix a personal lockout device to the key selector in that the key selector was not able to accept more than one key, and there were not other methods to facilitate group lockout during the work operations, on or about February 27, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
