29 CFR 1910.37(a)(3): Exit routes were not free and/or unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp were not provided where the exit route was not substantially level.   a) On or about February, 2024, along the back wall of the shop: employees were exposed to fire and entrapment hazards, in that; the exit sign above the paint booth door led to the interior of paint booth which was a dead end and used for storage.

29 CFR  1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a) On or about  February 27, 2024, in the warehouse and outside:  employees were exposed to struck-by and crushing hazards when operating the Yale Model GLC060TGNUAE084, Serial Number E187V07448V,  6000lb forklift, in that the forklift had extension sleeves placed over the forks without manufacturer written approval.   b)  On or about  February 27, 2024, in the warehouse: employees were exposed to struck-by and crushing hazards when operating the Yale Model GLC060TGNUAE084, Serial Number E187V07448V,  6000lb forklift, in that the forklift had a yellow man basket attachment that was placed over the forks without  manufacturer written approval.
