29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about 2/14/2024, during the installation of roofing material at a job site located at 2542 Wildflower LN NE, Massillon Ohio 44646; the employer did not ensure employees working on the roof of a residential single-family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material with air driven nail guns.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about 2/14/2024, during the installation of roofing material at a job site located at 2542 Wildflower LN NE, Massillon Ohio 44646; employees were exposed to a fall hazard of approximately 9.7 feet to the ground while working on a roof of a single-family home installing roofing material without the use of fall protection and or a fall protection system.
