29 CFR  1926.1203(h)(2)(ii)(A): Before entry operations begin, the controlling contractor must provide the information received from the host employer; to each entity entering a permit space and any other entity at the worksite whose activities could foreseeably result in a hazard in the permit space.  (a) 200 Clearwater Drive, Rock Falls, IL 61071 - On or about February 26, 2024, the employer, acting as a controlling contractor, did not provide the information received from the host employer; to each entity entering a permit space. Employees were exposed to the hazards associated with sewage, an oxygen deficient atmosphere, and unknown air containments upon entering a permit confined space (an approximate 11-foot deep sewer system manhole).  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
