OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  a) Jobsite: 5935 Woodland Avenue Philadelphia, PA 19143 - Loading Dock Area - On February 28, 2024, employees were working within the vicinity of an industrial steel racking system, used to store material to manufacture food products, with columns that was not anchored to the floor, exposing employees to unstable racking systems.

29 CFR  1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  a) Jobsite: 5935 Woodland Avenue Philadelphia, PA 19143 - Loading Dock Area - On or about February 28, 2024, the employer did not ensure that powered industrial truck operators had received an evaluation performances at least once every three years, thereby exposing employees to struck-by hazards.  Training shall consist of a combination of formal instruction, practical training, and evaluation of the operator's performance in the workplace.

29 CFR  1910.215(a)(1): Abrasive wheel(s) used on grinding machinery were not used with safety guard:  a) Jobsite: 5935 Woodland Avenue Philadelphia, PA 19143 - Maintenance Room - On or about February 28, 2024, the employer failed to ensure that employees who used grinding machinery were protected with safety guards.  Employees were sharpening knives and tools on the grinder without a guard in the maintenance room, exposing the employees to cut and abrasion hazards.

29 CFR  1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work:  a) Jobsite: 5935 Woodland Avenue Philadelphia, PA 19143 - Maintenance Room - On or about February 28, 2024, the employer failed to ensure that grinding machinery used by employees was equipped with a work rest to support offhand grinding work.  Employees who sharpened knives and tools on the grinder were not protected with work rest attachment for the grinder, exposing the employees to cut and abrasion hazards.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) Jobsite: 5935 Woodland Avenue Philadelphia, PA 19143 - On or about February 28, 2024, the employer did not provide effective hazardous chemical training for employees cleaning equipment.  Employees are exposed to chemicals, including but not limited to:  - 2-butoxyethanol and Sodium hydroxide contained in Chemsan 0115  - Alkyl dimethyl benzyl ammonium chloride, 1-IRcanaminium, N,N-climethyl-N-oclyl�, chloride, lloctyldimethylammonium chloride, 1-IRcanaminium, N-decyl-N,N-climethyl-, chloride, and Ethanol contained in 5032 degreaser - etc.

29 CFR  1910.305(g)(1)(ii): Flexible cords used as permitted in paragraphs (g)(1)(i)(C), (g)(1)(i)(F), or (g)(1)(i)(H) of this section were not energized from approved receptacle outlets:  a)	Jobsite: 5935 Woodland Avenue Philadelphia, PA 19143 - Maintenance Room and Production Floor - On or about February 28, 2024, the employer failed to ensure that employees were not using flexible cords in place of approved receptacle outlets.  Employees were utilizing a computer in the maintenance room, and a washer and dryer in the main production are, and they all were connected to a temporary extension cord being used as a permanent electrical source, exposing employees to electrical and fire hazards.
