29 CFR  1910.178(l)(3)(i)(M): The employer did not review the operating instructions, warnings, or precautions listed in the operator's manual for the types of vehicle that the employee is being trained to operate:   a) Aisle 39 - On or about February 20, 2024, the employer failed to adequately train employees operating Raymond High-Level Order Pickers in accordance with the operator's manual on the use of pallets, allowable pallet types, condition of pallets, and operator procedures.  An employee operating a Raymond High-Level Order Picker approximately 30 feet above ground level, while wearing a Personal Fall Protection Harness attached to a 3M lanyard, stepped onto a wooden single-faced pallet to place items from the pallet onto the shelf when the pallet slat(s) broke causing the employee to fall through the pallet and sustain a fractured left hip from the fall.
