29 CFR 1910.147(c)(6)(i)(C):The employer did not conduct periodic inspection which included a review, between the employer and each authorized employee, of that employee's responsibilities under the energy control procedure being evaluated: On or about February 29, 2024, the employer did not conduct periodic inspections which included a review between the employer and each authorized employee's responsibilities under the energy control procedures being evaluated. Employees working within the maintenance department were exposed to caught-in hazards while performing maintenance on the Jensen Express Blanket Machine (Machine Number 3M008) and applying Lockout/Tagout procedures to control hazardous energy. The employer did not conduct periodic inspections on employees which exposed employees to potential caught in hazards based on exposure to the release of hazardous energy.

29 CFR 1910.147(c)(7)(i):The employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls were acquired by employees. On or about February 29, 2024, the employer did not provide training to ensure that the purpose and function of the energy control program was understood by employees, regarding the clearing of jams on the Jensen Express Blanket Machine (Machine Number 3M008). Failure to ensure employees understand the purpose and function of the energy control program exposed employees to caught in hazards associated with clearing material from the feed section of the Jensen Express Blanket machine.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) covered the following elements, however, the actions were not followed as required by 29 CFR 1910.147 (d)(1)-(6): On or about February 29, 2024, the employer failed to ensure effective alternative protection was provided to protect employees performing unjamming activities on the Jensen Express Blanket Machine (Machine Number: 3M0008) located within the Clean Linen Department. Employees utilize interlocks to control hazardous energy while reaching into the Jensen Express Blanket machine to remove linen from the feed section. However, the interlocks failed to stop the equipment, exposing employees to potential caught in hazards based on proximity to the conveyors located inside the cabinet.
