Edgerton OH 43517: An evaluation of competency for employees operating Hyundai L.P.G model 25L-7A forklifts were not conducted and documented by the employer at least once every three years. The operating employees and others working around the forklifts were exposed to struck-by and/or crushing hazards.

29 CFR 1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color: a) Covington Box & Packaging Inc. On or about February 29, 2024 at the address of 407 West Railroad. Edgerton OH 43517: The employer did not ensure that hard-wired exit sign above main warehouse egress door was maintained and in good working order (sign did not illuminated during test). In the event of an emergency, exit egress may be hindered for employees attempting to exit the facility.
