OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards while operating a hydraulic cylinder (jack):  a) Work Floor - Employees were exposed to struck-by hazards while utilizing an Enerpac hydraulic cylinder to disassemble a machine. An unapproved 8-inch-long steel rod was placed between the cylinder and the machine to increase the length of the cylinder's ram. As the ram extended the steel bar ejected and struck an employee causing internal injuries which required hospitalization, on or about February 21, 2024.  No abatement�certification or�documentation required.

29 CFR  1910.179(j)(2): Overhead cranes were not inspected for defects on a daily basis for items including but not limited to; all functional operating mechanisms for maladjustment interfering with proper operation, deterioration or leakage of air or hydraulic systems, hooks, hoist chains, all functional operating mechanisms for excessive wear of components, rope reeving for noncompliance with manufacturer's recommendations:  a) West Bay, Assembly Crane - Frequent inspections were not conducted on a 10-Ton D.R. Cordell floor operated overhead bridge crane and associated hook, on or about February 29, 2024.  b) East Bay, Machine Shop Crane - Frequent inspections were not conducted on a 5-Ton Konecranes floor operated overhead bridge crane and associated hook, on or about February 29, 2024.   No abatement�certification or�documentation required.

29 CFR  1910.334(a)(2)(ii): Where there was a defect or evidence of damage that could expose an employee to injury, the defective or damaged item was not removed from service until the repairs and tests necessary to render the electric equipment safe had been made:  a) Work Floor - The power cord provided on the Enerpac hydraulic pump (Model PEM1521) had a damaged outer jacket exposing the inner conductors, on or about February 29, 2024.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1904.29(a): A log of all Work-Related Injury and Illnesses (OSHA Form 300), and/or the Summary of Work-Related Injury and Illnesses, (OSHA Form 300-A), and/or the Injury and Illness Report (OSHA Form 301) or equivalent forms were not kept by the establishment:    a) 11625 Anthony Highway, Waynesboro PA - On or about February 29, 2024, an OSHA Form 301 or equivalent was not completed for a work-related injury which occurred on February 21, 2024 and which resulted in "days away from work".   b) 11625 Anthony Highway, Waynesboro PA - On or about February 29, 2024, the employer did not keep an OSHA Form 300 or equivalent for a work-related injury which occurred on February 21, 2024 and which resulted in "days away from work".  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
