McComb, OH; the employer failed to effectively guard the line #9 laminators driven cross roll dough brakes. Employees tasked with flour dusting activities where dough passes through two horizontal driven rolls were exposed to amputation hazards.

If construction does not permit location at the back edge, the automatic stop bar or automatic stopping device was not located where it was most effective to accomplish the desired protection: a) Hearthside Food Solutions LLC, on or about February 25, 2024, at the address of 312 Rader Rd. McComb, OH; the employer did not ensure the line #9 laminator's Sheeter hopper was equipped with an automatic stop bar. Employees tasked with filling the flour sifter trough and performing flour dusting activities upon the platform were exposed to caught in hazards created by the two ingoing horizontal rollers of the sheeter.
