29 CFR 1926.1404(h)(2): The size, amount, condition and method of stacking the blocking was not sufficient to sustain the loads and maintain stability. On or about February 29, 2024, the employer did not ensure that the Simon-RO Boom Truck (Model: TC4792 / Serial: 4900795251) was set up securely and properly leveled. The crane's outriggers were supported by wooden boards of various sizes which did not provide firm support. This condition exposed employees to crane collapse hazards.

29 CFR 1926.1427(a)(1): Operation during training. An employee who has not been certified/licensed and evaluated to operate assigned equipment in accordance with this section, operated the equipment as an operator-in-training not under supervision in accordance with the requirements of paragraph (b) of this section. On or about February 29, 2024, the employer did not ensure that the operator of the Simon-RO Boom Truck (Model: TC4792 / Serial: 4900795251) was qualified to operate the truck crane by being trained and certified by this standard. The employee was not accompanied by a supervisor or qualified operator. This condition exposed the employee and subcontract employees to struck-by hazards.
