29 CFR 1910.29(b)(2)(i): The employer did not ensure that guardrails met the requirement that midrails that are installed at a height midway between the top edge of the guardrail system and the walking-working surface. (a) Norfolk Southern Railroad, Maintenance Bay, Northwest stairs; on or about 02/28/2024, and at times prior, the stairs used to gain access to the second floor used for accessing parked locomotives for servicing had a top rail but was missing the mid-rail on the open side of the stairs creating a fall hazard.
