Employees engaged in buffering, deburring activities in the Main Machine Shop and Gearbox Shop were exposed to health hazards in that the employer had not determined, through a medical evaluation, the employee's ability to use a respirator before the employee is fit tested or required to use a respirator. Employees engaged in fabrication, machining, grinding, and sharping activities using the following grinders in the main machine shop and the gearbox shop were exposed to caught-in hazards in that the work rest opening were not maintained at a maximum of one-eight inch. a) Baldor Grinders/Buffers 1022W, serial number F275 in the machine shop, the work rest was found to be approximately five-sixteenths inch away from the grinding wheel. b) Baldor Grinders-Buffers 8107W, serial number 389 in the machine shop, the work rest was found to be approximately five-sixteenths inch away from the grinding wheel. c) Dewalt Heavy Duty 8" Bench Grinder DW758, serial number 00240-YL0427 in the gearbox shop, the work rest was found to be approximately three-sixteenths inch away from the grinding wheel.

Employees engaged in fabrication, machining, grinding, and sharping activities using the following grinders in the main machine shop and the gearbox shop were exposed to caught-in hazards in that the adjustable tongue guard opening were not maintained at a maximum of one-fourth inch. a) Baldor Grinders/Buffers 1022W, serial number F275 in the machine shop, the distance from the abrasive wheel to the adjustable tongue guard was found to be approximately 1.5 inches. b) Baldor Grinders-Buffers 8107W, serial number 389 in the machine shop, the distance from the abrasive wheel to the adjustable tongue guard was found to be approximately 1.25 inches. c) Baldor Grinders-Buffers 8107W, serial number 389 in the machine shop, the distance from the abrasive wheel to the adjustable tongue guard was found to be approximately 1.0 inches. d) Dewalt Heavy Duty 8" Bench Grinder DW758, serial number 00240-YL0427 in the gearbox shop, the distance from the abrasive wheel to the adjustable tongue guard was found to be approximately 1 and 5/8 inches. e) Dewalt Heavy Duty 8" Bench Grinder DW758, serial number 00240-YL0427 in the gearbox shop, the distance from the abrasive wheel to the adjustable tongue guard was found to be approximately ninth-sixteenth inches.
