29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: (a) 701 Stoney Battery Road, Landisville, Pennsylvania - None of the energy control procedures for the McKinley 66-inch jumbo flexo-folder-gluer (employer machine center 329) included dissipating mechanical energy, such as the rotating counter-ejector delivery rollers, which coast to a stop after activation of an electrical off switch, exposing employees to caught-in and entanglement hazards, on or about February 21, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal, and transfer of lockout devices or tagout devices and the responsibility for them: (a) 701 Stoney Battery Road, Landisville, Pennsylvania - None of the energy control procedures, including, but not limited to, those for the McKinley 66-inch jumbo flexo-folder-gluer (employer machine center 329), specified the procedures to utilize for group lockout/tagout, exposing employees to hazards such as caught-in, struck-by, and entanglement, on or about February 21, 2024. (b) 701 Stoney Battery Road, Landisville, Pennsylvania - None of the energy control procedures, including, but not limited to, those for the McKinley 66-inch jumbo flexo-folder-gluer (employer machine center 329), specified the procedures to utilize during shift or personnel changes to ensure the continuity of lockout/tagout protection, including provision for the orderly transfer of lockout/tagout device protection between off-going and on-coming employees, exposing employees to hazards such as caught-in, struck-by, and entanglement, on or about February 21, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
