29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) Jobsite located at or near 4701 Pine St., Philadelphia PA- On or about February 29, 2024, the employer did not train each employee conducting masonry activities on scaffolds, who were exposed to fall hazards between approximately 59 feet, to recognize the hazards associated with working from scaffolds to understand the procedures to control or minimize those hazards. b) Jobsite located at or near 4701 Pine St., Philadelphia PA- On or about February 29, 2024, the employer did not train each employee conducting masonry activities on scaffolds, who were exposed to fall hazards between approximately 53 feet, to recognize the hazards associated with working from scaffolds to understand the procedures to control or minimize those hazards.

Employees conducting masonry activities were utilizing a scaffold platform that was not fully planked, thereby exposing an employee to fall hazards or approximately 59 feet. b) Jobsite located at or near 4701 Pine St., Philadelphia PA- Exterior Large Scaffold- On or about February 29, 2024, the employer did not ensure that each working level of a scaffold was fully planked or decked between the front uprights and guardrail supports.
