29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   A) 47 Boplar Street, New Milford, Connecticut carpentry and roofing project: On or about March 1, 2024, employees were exposed to a fall hazard of an approximate height of 56 feet from the ridge of the roof to the ground while installing roof trusses without fall protection on a 10/12 pitch roof.  B) 47 Boplar Street, New Milford, Connecticut carpentry and roofing project: On or about March 1, 2024, employees were not protected by the use of guardrail systems, safety net systems, or personal fall arrest systems of a rough opening to install a window/sliding door, exposing them to a fall hazard of an approximate height of 30 feet to the ground.

29 CFR  1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.  47 Boplar Street, New Milford, Connecticut carpentry and roofing project: On or about March 1, 2024, midrails were not installed on the rough openings of the building to install window/sliding doors, exposing employees to fall hazards with the following:  A) No midrails installed on the first floor, exposing employees to a fall hazard of an approximate height of 13 feet.  B) No midrails installed on the second floor, exposing employees to a fall hazard of an approximate height of 24 feet.  C) No midrails installed on the third floor, exposing employees to a fall hazard of an approximate height of 32 feet.  D) No midrails installed on the fourth floor, exposing employees to a fall hazard of an approximate height of 44 feet.

29 CFR  1926.502(b)(5): Midrails, screens, mesh, intermediate vertical members, solid panels, and equivalent structural members were not capable of withstanding, without failure, a force of at least 150 pounds (666 N) applied in any downward or outward direction at any point along the midrail or other member.  47 Boplar Street, New Milford, Connecticut carpentry and roofing project: On or about March 1, 2024, the midrial located on the landing of the straight staircase that is used as an access point to enter and exit the building was not properly seated and secured, exposing employees to a fall hazard of an approximate height of 12 feet while transporting tools and construction materials.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  47 Boplar Street, New Milford, Connecticut carpentry and roofing project: On or about March 1, 2024, employees were using a Werner A Frame ladder to gain access to the next floor as the side rails did not reach the landing surface leaving a gap of approximately 4 feet exposing them to a fall hazard of approximately 11 feet from the landing surface to the ground.
