29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Lowes Home Centers LLC. - Warehouse and back storage area: A Forklift carpet extension attachment was used on the Propane YALE lift truck Model # GLP050LXNVAE091, S/N A974V03326L and the propane YALE lift truck Model # GLP050LXNVAE091, S/N A974V03316L to handle and move rolls of carpet throughout the back storage area and the store. Written approval for the use of the fork carpet extension was not obtained from the forklift manufacturer, on or about 02/29/2024.
