29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  (a) Residential construction located at 735 Stella's Pointe McDonough, GA; roof: On or about March 4, 2024, and at times prior, the employer exposed employees to struck-by hazards while operating framing nail guns, in that protective eyewear was not provided to employees operating these tools at 120 PSI while engaged in framing activities.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) At 735 Stella's Point, McDonough, GA 30253; on roof, on or about March 4, 2024 employees performing roofing operations were exposed to fall hazards of approximately 10-feet from the lower landing and approximately 24-feet from the roof to the lower level in that the employer did not enforce the use of personal fall protection.  General Construction Bethel LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1644456, citation number 1, item number 1, and was affirmed as a final order on 3-6-2023, with respect to a workplace located at  520 Country Ridge Drive Hoschton, Georgia.

29 CFR  1926.552(a)(4): Hoisting ropes were not installed in accordance with the wire rope manufacturers' recommendations.  a) Residential construction located at 735 Stella's Pointe, McDonough, GA; right side of building: On or about March 4, 2024 the employer exposed employees to struck-by hazards while operating a Material ladder hoist , in that the manufactured installed wire rope was replaced by a nylon climbing rope, which is not a proper replacement cable as indicated by the manufacturer of the material hoist equipment.
