29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about March 4, 2024, and at times prior thereto, employees on all sides of the construction site engaged in roofing activities without fall protection were exposed to unprotected fall hazards over 6 feet at all locations.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about March 4, 2024, and at times prior thereto, employees were accessing a one story roof upper landing surface and did not extend the ladder 3 feet about the roof line exposing the employees to fall hazards.
